Insurance Plans
We participate in most major insurance plans, including but not limited to the following, BCBS, Cigna, Atena, UnitedHealthCare, PHCS, GreatWest, Humana, Medicare, Medicaid, etc.
We will file your charges to the insurances on your behalf for treatment received from the our office. Lab tests sent out will be billed to your insurance directly by the lab performing the test. We will be happy to assist you with billing questions.
Co-Payments
Co-payments are required to be paid in full with each office visit, as set by your insurance plan. You are required to pay your co-pay in full before you see the provider or before any service is rendered.
Insurance FYI
We accept most insurance plans.Insurance coverage however varies from contract to contract. Employers may change insurance coverage annually. Please remember to bring your insurance card to every visit to ensure we update your information. It is also your responsibility to contact your insurance to verify your benefits and also if we are a participating provider. You will be responsible for co-pay and deductibles at each visit to our office.
Referrals
For services not rendered at our office we refer our patients to exceptional offices and facilities and we request at least 48 hours notice for processing referrals, except in emergency situations. After you have been seen for your referral we will discuss your report with you at your visit.
Past Due Accounts
If there is a past due balance on your account and you have been sent a notice to pay, you may call to make payment arrangements. If no payment arrangements are made and the account remains delinquent, your account may be referred to an independent collection agency.
